CAMOUFLAGE CONSENT FORMS

Here are copies of all the Consent Forms that you will require. Please feel free to download these and print them off.
A consent form must be completed for each participant between the ages of 12 and 18 and must be signed by their parent or guardian.

Failure to produce a signed consent form on or before the day of your activity will result in exclusion from the day.
